What is an Orthopaedic Surgeon?

An Orthopaedic Surgeon is a medical doctor who specializes in the treatment of musculoskeletal system, including bones, joints, muscles, ligaments, tendons, nerves and skin.

What is a PA-C and a NP?

A PA-C is a Physician Assistant-Certified. An NP is a Nurse Practitioner. Raleigh Orthopaedic Clinic using a private PA/NP model, and the midlevel providers work directly with a doctor.

How do I schedule an appointment?

How do I make an initial appointment for a workers compensation claim?

The patient needs to contact their employer or case manager, and the employer or case manager will call to make the initial appointment.

If I have already been treated by another orthopaedic practice, what do I need to do to make an appointment at Raleigh Orthopaedic?

Please call the appointments line (919) 863-6808 and determination will be made whether you will need to provide physician’s notes, surgery notes, x-rays, etc. for review prior to making the appointment.

What should I bring to my appointment?

You will be asked to fill out medical history forms. To assist you at the time of your appointment, you may want to bring a list of medications, any previous surgery information, including the date of the surgery. If you wish, you can click here to download the patient forms , complete at home and bring it to your appointment.

  • Copay (Your insurance card will indicate what the copay amount is for your plan)
  • Insurance Card
  • Photo Identification
  • X-rays or MRIs that have been taken at a facility other than Raleigh Orthopaedic Clinic.
  • If you are under the age of 18, you must be accompanied by your parent or legal guardian
  • If you are unable to complete the medical history forms, please be sure to bring assistance or click here to download the patient information forms to complete them prior to your visit.

We are open Monday through Friday from 8:00 AM until 5:00 PM.

We have 3 locations, including an Athletic Performance Center throughout the Triangle region:

Is a co-pay required for follow up visits?

Your insurance policy determines whether or not you have a co-pay for your follow up visits; however, under most insurance policies, a co-pay is due at each visit.

What is the best way to request medication?

The fastest and most efficient way for you to help us take care of your medication refill requests is to have your pharmacy call us or fax your medication request to (919) 863-6904.

How do I get my short-term disability or Family Medical Leave of Absence forms completed?

These forms are usually provided by your employer or insurance company. In an effort to streamline the process of completing the large volume of forms we receive daily, we are unable to complete short-term and long-term carrier-specific forms. Instead, we will attach to your form a computer-generated form, which will provide all the information that your insurance carrier is requesting. It may take up to 5-7 business days to process your form. There are several ways you can get these forms to the administrative assistant for your doctor or mid-level provider.

What should I wear to my physcial therapy appointment?

Sweat pants, exercise outfits, ect. If you are coming from work we have facilicites where you will be able to change.
